BCM400, T1, only 5 of 24 lines show up

I am installing a new BCM400, ver 4.0 with current patches. It is replacing a Norstar with Access Advantage lines coming into slot 1. I'm putting this circuit into a DTM and have verified that all the lines are provisioned. However, of all 24 lines, only the last 5 will work, and only if I assign them to a set and grab them directly. when dialing "9" I get dead air and can only dial5 more digits before the phone goes idle. If I try to grab any other line the display says "line in use", and goes idle.
All lines match the outgoing Norstar, Loop start, Manual answer... Whats with the last 5 lines?

I've always used E&M, Auto Answer on straight T1's into a DTM card.

Found the trouble, duh... Customer informed me today that all lines on that T1 are configured as Ground start, plus only 19 of the 24 are active. Completely contrary to what I found in the Norstar it replaced...

Amazing what works when settings are correct!!!
