BCM400 ASM port locking

I am an IVR programmer and am having problems at 2 locations with BCM400s. We have the exact equipment at 2 other locations that work great.
The first problem is that we have to inline wire analog phones to the ports to get the IVR to detect the ring (Dialogic D120jctls).
The second problem is that intermittently the ports have to be reset by picking up the inline phone and hanging it up. There is no consistency on how or when this happens. The caller hears ringing but the ring never gets through to the IVR. 2 locations have these EXACT problems. The other 2 locations have NO problems (Same equipment(hardware)).

Any thoughts?
 
Are the patch levels the same on the system? There was a core patch that came out a while back that dealt with issues on the ASM cards - don't recall exactly what the issues were though at this point.
 
any idea where I can find release/build notes for patches without having to access Nortel website?
 
looks like the 'good' system has 3.6 cumulative 2 patch. The 'bad' system has 3.6 cumulative 3 patch. Problem is, to 'rollback' the patch will be a massive undertaking.
Could the 'fix' for the ASM have been wiped out in the cumulative 3 patch-how can I find out where that was fixed?
