BCM IP Talk Path / No Voice Passthrough

I have a BCM 400 3.7 with several IP clients around the US
all users can connect fine and register some are experincg no talk path or phones losing talk path after setting idle for some time. All users are connecting via cable or DSL and we have a mixture of I2002 and I2004 phones in use. Any insight would be helpfull.

Ports 7000 UDP / 28000-28255 UDP / 51000-51200 UDP are passed through to the BCM 400
