BCM 50 3.0. Installed last week. Has all patches we currently have. System has 8 Port Trunk module in expansion. System ran for week. Then yesterday Trunk Module became unavailable. Didn't see it in programming. Reset system multiple times. System would not start up. Replaced with MICS and Call Pilot to send in BCM. Later yesterday tried powering up BCM and it did come up for a while. Without phone plugged in. Went into alarms and it had this error and previous alarms were GONE. Time was still there, programming was there but previous alarms were not there. Plugged in telephone into port 401. And kaput, system went down again. I'm thinking just OOB failure, but figured if anyone has come across this.
