BCM 50 Expansion MBM not coming up

Installed the keycodes and expansion 2 on the system. (Exp 1 is a working DSM 16). Configured the MBM as a CTM4/GATM4. No problem. In Element Manager the module is always in 'Enabling' status. Have disabled and re-enabled a few times; also rebooted main cabinet and Expansions. It is the patch cord that comes with the module. When I access the trunks from a phone (095-098) I just get a fast busy and 'not in service' on the display. Any ideas?

Make sure all the dipswitches are set to "on" on your GATM4.
You plugged in your second expansion patch cord to the 4th connector on the Bcm50(far right)?

I Have had a lot of trouble with cards not downloading the firmware to the card when I do an out of box install. I take the card & put it in a BCM 400 to get it's firmware & than stick it back in the BCM 50.
