BCM 450: ASM 8 enabling

BCM 450 all of a sudden my ASM 8 in the main cabinet shows 'enabling' No dial tone the ports show 'unequipped' I have not been on site yet but I did software disable the card and re-ebable. No change. I then had customer power down the main cabinet. No change. Any ideas? I know it is not recommended to remove card with power on but I have heard from many techs you can reseat an ASM while system is up

Check for alarms and the status lights on the ASM module. As David wrote, it sounds like the ASM has failed.

OK i replaced ASM and the phones came up. Then a couple days later phones went down and card shows 'Enabling' and ports 'Unequipped' Another bad ASM? Seems unlikely what am I missing?

A few other things to check: Check the alarm log for clues, see if the latest software patches are installed, replace the expansion cable, replace the expansion carrier's power supply, replace the expansion carrier.
