BCM 400 echo on PRI

We have a new install of a BCM400 (3.7) served by a CLEC PRI. The PRI is connecting via TDM to a MetaSwitch at the CO. Every now and again there is an echo on inbound calls. Sometimes it's on outbound.

Both the CLEC and the equipment vendor say they've checked everything they can imagine. Does this sound familiar to anyone? How did you solve it? Any suggestions on where to look?
 
Sorry, should have mentioned that. No. All phones are digital. There is no IP in the current location, although we will be using IP trunking to connect 10 remote locations in the future.
 
Try changing the LBO on the internal CSU to 7.5db - see if that helps. Could be the circuit is too "hot" which is causing the echo. If 7.5db doesn't change anything, try going to 15db.
