BCM 400 dropping voice path

I have a BCM 400 4.0 system installed for nearly 3 years. Never had any issues and it is patched up to the Feb 2010 cumulative. The original report was that the system is dropping calls randomly. Upon investigation I believe the voice path only is dropping out and one party then hangs up. This system has VoIP trunks back to a CS1000. 9 other BCM's are hooked up same way installed at same time and have had no other reports of problems. Users have said calls within the building are dropping too (not using VoIP trunks) Any ideas?
 
Forgot to mention I have moved a couple users to a different DSM32+ card with no change. This is a school environment so 3-4 phones handle 90% of the calls.
 
Are all phones having problems or just the 3-4 that handle 90% of the calls? Are headsets being used? Have phone handset receivers been checked? What does BCM monitor show?

The other phones have not reported problems but they are hardly ever on the phone (classrooms)

No headsets.
Entire phones including handsets have been replaced for the 3 initial reported sets. Then they were moved to another port on another DSM32.

I havent seen anything on BCM monitor but I have not been able to watch it that long since I have many other school districts to take care of.
