BCM 400 4.0 Losing DN programming

I have had a couple of my sites (3 of them) that have lost their DN programming after a power loss, and One of the site this has happened 2 different times. The really stange thing is that the remainder of the Telephony Programming is still there (buttons, hunt groups, even the name of the Extension sit still there! etc..) and the Call Pilot is all in tact, just the DNs revert to the default DN, we go in and restore the data base and all is good again, but why is this happening? is there a hardware failure or a software patch that is needed
 
I wish I had something useful but this happens to me too. It seems to be a 'feature' of the system after power failure. I just do a core telephony restore when it happens.

Description (Problem Clarification)


BCM (Business Communications Manager) Release 4.0 has an issue due to unexpected power failure, as it happened in the site that power goes off without having the chance to shutdown the BCM, BCM goes back to the default configuration.
When trying to apply the system patch to prevent recurrence, the system patch (BCM.R400.SU.System.026-201002) could not be applied and the following error is seen:
'Software upgrade failed to apply, rpm retcode 57.'

Resolution Plan


This fix requires use of the nnsupport account and the password of the day which can only be generated by Avaya Technical Support.
Please contact Avaya Technical Support and refer to solution id9445862.

 
So if i load this patch, there is a chance that i will get this error. So my next question would be, If this patch fails, does the system continue to work or do i have a down system while i contact Avaya?
 
First load latest pacht and probe..
if continues you have 2 options
1.- contact Avaya
2.- Reset to default (level 2 or Re-image) and reload keycodes,patches and restore of previuos backup
