BCM 4.0 on hold issues

Call A comes in to the receptionist, she answers and puts it on hold. Intermittently,(of course) call B comes in, she puts that on hold and calls a and b are conferenced. Anyone had this one?

Fat fingering the calls. She's got caller 2 on the line and pressing call 1 without first pressing hold.

Voila, one conference call.

She might have a button sticking intermittently.

Have you tried replacing her phone to test it?

Patch it with the oo8 patch. We had an issue with one of our customers that was having the same issue on individuals phones. We applied the 008 patch and the issue went away.
