BCM 1000 with expansion unit can not hear audio on one side of call.

I was wondering if this ever got resolved. I am having the same problem with only two ext. I have swapped out the phones, the dsm's around which solved the problem for a year. I have replaced cable, jacks, change to open ports, etc. But it always seems to go back to these same locations. Its like they are possesed. This is unit is a BCM 1000 running 4.0 something with an expansion unit. The only thing I havent done is swap the dsm's from expansion unit to main unit. Any help would be appreciated.
 
-Is the system patched to the latest? if not patch it before we carry on.
-Swap the DSM's asap
-Are the 2 exts on main or exp cabinet?
-Have you tried moving those 2 users to another station port on the BCM?
-Replaced/re-punch jacks and closet end of cable?
-What type of lines?
-What type of phones?

2. I've swapped the dsm's in the expansion unit with each other. But not with the ones in main unit. That will be done this Friday the 19th.

6. To explain this set up location. Goes from unit thru amphenol connector to 110 block. Then is cross connected to another 110 block to a 25 pair. Then that 25 pair runs thru ceiling to a gas blocked 66 block. Then runs thru conduit to a cabinet that houses another gas block 66 block. Then it runs to a patch panel. That is patched with cat 5e cords to the user end patch panel which uses cat5e 2cable to a jack that the user is attached to. The user end wiring, jack patch panel, gas blocks at both locations have been replaced. I have connected the phone directly to where the am phenol makes its connection to the 110 block and still have the problem. So it brings it down to being am phenol connector and dsm's.
