Bad Media on version 8.6

I am getting a <bad Media> message no matter what tape I put in the DAT drive. I tried erasing the tape using ntbackup but am getting unable to rewind tape hardware failure message from the device. Would this mean the DAT is knackered? Is ther also an issue with the media pools within veritas?

I also got a Bad Media message, inserting an Arcserve-formatted Ultrium tape. Erasing the tape rendered it usable for BE 8.6. Other AS-initialized tapes worked fine in Bexec.
I don't know if this is a miracle, but it worked!
Please double-check your scsi chain, IDs, cables and termination. It may be some hardware problem, or a dirty tape drive.
