Avaya Session Manager traceSM "Rejected - 500 Server Link M>"

I've searched for what Rejected - 500 Server Link M> means. I have went through everything in the routing on SM but I keep ending up with Rejected - 500 Server Link M> when trying to place a Conference Call into Lync 2013. We have looked at several setup guides for Lync 2013 and Session Manager. If anyone has seen the traceSM error, please let me know what it might be. Thank you

There is no other information in the traceSM. It only indicates it was rejected.

Basically that's SM telling the thing trying to get to Lync that SMs monitoring state for the Lync entity is down for whatever reason.
