Avaya Model B159 Polycom device

My customer has a Nortel CS1000 VOIP server 5.5. We are using an Avaya Model-B159 Polycom analog device for teleconference purposes in a large conference room. This device is on a MGC card/analog TN on the CS1000 server. In the morning when she dials the conference number by depressing the green key for dial tone and dials 9, receives a second dial tone, 1 area code and 7 digits on the touchtone pad. The call does not go thru. She does not hear musical tones from the touch tone pad and the call disappears. She tries at least 8-10 times and then the call goes thru.
Our technicians are receiving dial tone from an analog phone when testing. They are also able to complete calls from this device. HELP?

If it works from an analog phone as you say then I would replace the Polycom.
 
@KCFLHRC - Wrong. Avaya phone made by Konftel which is owned by Avaya.
 
Well there ya go, who ever makes it I would get a new one.
