Avaya Index 400 Issue

Having a problem with the Index 400. We have one extension which is losing calls on a regular basis when the user picks up the phone. Phone logs show that the call clear is normal. Have replaced handset, wiring and card that extension resides on. If another user within the pickup group picks up a call on that extension they sometimes lose the call too.

Anyone had a similar problem? Any ideas?
 
I would try defaulting and then deleting the user record completely. Then re-create the same user id, or create a new user/ext id for that person.
