Avaya Call assistant (one X app) Dialing rules

Has anyone had the problem where the dialing rules are being ignored?

9 to dial outside
1 country code
added my city code
no PBX prefix
1 for long dist
011 for out of the country
internal ext 4 digits
10 digit US numbers.

Doesn't dial 9 when I use the dial feature. I've reinstalled this app twice, but I cannot get the dialing rules to work.
