Avaya 1408 phone lost connection

Avaya digital phone 1408 disconnects and stuck in rebooting from time to time.

To bring back the 1408 phone online, I had to reboot the PBX.

We recently upgraded the avaya IP office 500v2 from R9.1 to R11.1 and this starts to happen. Any idea?

The version is: IP office 500 V2 Essential edition R11.1.
 
Try switching the 1408 with 1408 at another extension. If it follows the phone replace the phone. If it stays with the port replace the card. If it goes away chalk it up to being Avaya.

The truth is just an excuse for lack of imagination.
 
We already changed the card but no luck.

All 1408 phones are affected.
 
Do they run the correct firmware for that release?
Maybe they try to upgrade something and can't get the files.
