Automatic email upon overdue

I was wondering if it was possible to automaticly email someone if the report was over 30 days old? The main problem I am having is the email address of the responsible person is entered on the form. How can I take that address and send a message to this person?

How will we know if the report is over 30 days old? I am going to first assume there is a date field that we will use as a point of reference. Is that field on the form or the report? Is the email address already part of the database? ljprodev@yahoo.com

Yes there is an initail start date that the evaluation was opened. Sorry about that.
