Autofilling multiple fields

I have a table (1) which has a look up field in another table. The look up table also contains additional info such as phone numbers etc. What I would like to be able to do is to autofill related fields in table (1) with information from table (2) when I select the individuals name.

Are you wanting to do this through a form? Because if not, I am not sure it is possible.
