Authentication Failures

Has anyone experienced problems with NMS 9.2 listing alarms for Authentication Failures on Bay Routers. I have 30 routers of which NMS is reporting this problem on 20 of them. I have checked the community table - correct, checked the logs on the routers via site manager - nothing listed for authentication failures. Running software version 14 on them, also experience the problem where it locks out the telnet port.

Can definately say that the routers aren't being hacked.

any ideas????????
