audio resource question

By now everyone has seen the problem of 1 way, no way audio - typically my experience has been to prove the route (and/or ports) for the RTP stream thru the network from phone to phone.

But I always get offhook dialtone, despite the fact that I have no audio stream from a medpro or voip module...? I know its source is probably local, but where is it coming from - in the old TDM world, I'd say it's coming from the timeslot - but what about the new IP enabled stuff like the 450 gateways?... where's the dialtone coming from on a call where the phone isn't getting an audio path but still hear's offhook and ringing?
