Asterisk Call notification

We need to find an application that can take the inbound callers ID and perform a lookup in MS SQL then display that information on the agents computer screen as a screen pop. I have seen a lot of programs that will use MySQL but nothing for MS SQL. Does anyone know of a aplication development company that can do this? Ideally we would like to have the application semi-customizable so we can add additional fields displaying information from the MS SQL DB.

I would look at AstAssistant. It is a good small app that can be customized to lookup the callerid via a URL.
