Assessing skills....
As in a checklist for your own professional development to help identify weak points? To critique other (or potential) instructors, like an evaluation sheet? Or do you mean a student evaluation either for self improvement or to help the training center track instructor performance?
There are resources available for all of these. I'll post some URLs when I have a better understanding of what you're looking for.
