asp rating app

I am looking to put a simple rating app (ie. a user rates a site from 1-5, and results are queried and shown as stars, or other). I've searched high and low. I found some that use an Access db but nothing that i can easily implement using asp/mysql. any assistance, or a pointing the right direction, would be greatly appreciated.

Anything that uses access should be relatively simple to convert to use mysql instead. Only the connection strings and sql statements should need to be modified.
