ASM problem

I have a mICS 6.1 with a 6 port fiber. I connect an ASM to the fiber, reboot both to be safe, set it as an ASM in hardware (module 3) but can never get a dialtone from the ASM. Fiber card and fiber cable are good, so is the 25 pair. THe ASM has as solid green light. Do I have a bad ASM or have I made some mistake in installation.

I've seen a good bunch of bad ASM with flashing power lights, but not this one.
 
Does Maintenance/Port Status show each port as an ATA?
If not:
Try another fiber cord

Can the port receive an internal call and/or has Talk Battery?
If so:
Did you program a couple Intercom keys and prime to Intercom?
or if Prime to Pool X did you set Hotline to external with a pause (F78)?

I tried a 0x16 with the fiber cards and cords and they come up perfectly.

A different fiber cord, fiber card and 25 pair cable proves it must be the ASM. No talk battery, nor does it show up in maintenance, nor does the light flash, it's solid green. It's been on my shelf for a while to double check.
 
Never opened one up before but wonder if something silly like a ribbon cable from PS to Mobo has come loose....if there is one.
