Appearance buttons not alerting

Our reception phones have three appearance buttons to handle incoming calls. When they are on the phone and another call comes in they used to see the call on the 2nd appearance button, put the first call on hold, then pickup the second call. Lately the second appearance button will not alert them for new calls coming in. They can hear the phones ringing at other extensions so they put their current call on hold and have to wait about a ring or two before the call appears on their phone. Is there a setting that could cause this to happen?
 
Is call waiting enabled for the hunt group and/or user?
 
Call waiting on" is unchecked at the user level.
 
Check it (turn on) for the group, it has no effect at user level with multiple appearences :)

Per the docs

On pre-IP Office 4.0 systems, all the group members must have their own call waiting setting switched on, this no longer applies for IP Office 4.0+.
 
Indeed, I just assumed this was 4.0 + :)

If I set up a separate line appearance button for each incoming line in addition to the three appearance buttons, the receptionist can catch the second call coming in. The alerting call then shows on both the appearance and line appearance button. Strange.
