Apache not working with AD 2003

We upgraded Active Directory 2000 to Active Directory 2003 and now our Apache LDAP process fails with the [error] mod_ibm_ldap: failed to search '???? ?????'
with filter '(sAMAccountName=????????)': Operations error. I tried turning the logging level to debug but I still do not get any additional errors. Any idea how I can get more information in the logs or trace this problem so I can determine why it is failing? (running IBM_HTTP_SERVER/1.3.26.1 Apache/1.3.26 (Unix)

Did you uncomment the lines
# LoadModule ibm_ldap_module /path/to/mod_ibm_ldap.so
and
# AddModule mod_ibm_ldap.c
in your httpd.conf?

In the httpd.conf file the line
LoadModule ibm_ldap_module libexec/mod_ibm_ldap.so is uncommented, but the # AddModule mod_ibm_ldap.c is not.
