Anyone have a newer edition of programming sheets on NTP's?

Hi, I'm looking for the standard Nortel Meridian programming sheets.

They're the ones that come with the NTP's when you first attend class.

They look like an Excel Document, say the LD #, then like:

REQ (NEW, OUT, CHG, ETC.)
TYPE (500, 3904, SL1, ETC.)
TN
