anyone else having this problem with voicemail and mailbox 12?

Partner ACS R5 processors on both systems.
I have had two different systems have this problem to the past three months.
one system is a VS mail and the other was a PCIA card both are mailbox 12.
I am having the problem that no one can leave a msg at ext 12 (saying the mailbox is full and no lights are on.) I have had everyone clear their boxes and can leave msg's on other boxes no problem.
I can't log into MB12 to tells me my password is incorrect.
I have reset the MB password still tells me my password is incorrect.
I have deleted the MB and re-installed and the password is incorrect.
I have removed the bad MB first and gave what I thought a new MB20 to a new ext and then could get into the new MB20.
I then deleted that and tried MB12 again and still can't get in.
The one system I just gave the user a new EXT but that will not work in the other system.
Any ideas on what is wrong or to try?
 
Focusing on the Partner Voice Messaging...which mailbox is assigned to x12?...keeping in mind that mailboxes are numbered 1 through 4 or 16...depending upon which version you have (small or large).

If you have an available mailbox, you could assign it to x12.

There have been rare instances where the voice mail box greetings are overwritten by system prompts.

Lastly...you could default the PCMCIA and just reprogram from scratch.

There may be other things to check and the lead members will offer up more suggestions. [smile]

On the PCIA card it is a "small" canned 2x4 style.
I did factory default it and I still can't use 12 or in this case it didn't have any effect.
I removed it from box 4 and changed it to 3 and put ext11 on 4 and I can access ext11 mbox but not 12 says invalid password.

VS mail
All 20 boxes are in use.
I deleted MB12. bringing it to only 19 boxes in use.
Then assigned an new ext40 to the open slot bringing it back to 20 in use. I could access the new box created.
I then deleted another MB13. back to 19 in use.
I assigned that open to MB12. Again I get Password is invalid. Reset password no luck.
Just weird.....
 
Just to clarify...by factory default, you completely wiped the voicemail and re-recorded/provisioned everything from scratch?

Do you have a spare license card for the VS? These things were bad about being corrupt when they were new from Lucent/Avaya and now it's had God knows what done to it. You can buy a used 2 port card on ebay right for $20 or best offer so that would be a cheap test.
