Any known issues with 4.1.9 regarding misdailing on line appearance?

Does anyone know about issues with line appearance access and misdialed phone numbers? The system is an IP500 4.1.9 with 2410's. This issue is intermittent and does seem to happen randomly. The system is using analog lines and was attempted to be programmed as a key system. Each phone is programmed with one call appearance button and the rest are line appearances. I am not the original programmer, just trying to clean up someone else's mess (a different BP). If I had programmed this system I would not have opted for line appearance access as it just causes issues with the phone system and gives the customer a headache (and me).
