Analog Stations on Nortel ICS 032 Release 5

I recently installed an ICS 032 with 4 016 modules, 2 8 Port analog modules, 1 Trunk module, 1 T-1 card (PRI). The system was turned off. After it restarted the analog ports didn't work properly. You could call to the extensions but you wouldn't get a dial tone if you go off hook on the analog ports. Turning the modules off and turning it back on restored the ports. The next day a couple of the ports had the same problem. By disabling the ports and then enabling them the problem was corrected. A couple of weeks later another analog port had the same problem. Another suggestions on why this is happening? And of course what to do to fix it.
 
Appart from glitchy Rls 5 software... what kind of TE do you have on the analog ports? Is it the same ports having the trouble?

One of the Ports is connected to an Computer via a Internal modem card. One of the ports is connected to a VMI that is going to a Confer II Conferencing bridge.

So is it the same ports having the troubles? If so move the TE to other ports. If the problem follows the TE, then the Analog Mod may not be compatable with that kind of TE. Not all phone devices have the same impedance off hook or on hook. You may have to intervene on the analog circuit side to meet normal TE characteristics... just stray thoughts.
