All lines start to flash when there is multiple calls 3

When there is about 3 or 4 calls in play all of a sudden when another call comes in all the rest of the lines will flash as if on hold and some current calls will drop.
The last caller trying to call will hear just a hangup click.
When you press the flashing line it's a busy tone or try your number again.

This issue is intermittent but will happen every 10 minutes or so when they are busy.

This started on an 8X24 which was replaced by a MICS but has the same issue still.

All lines register ring voltage at about 83v

I am thinking something with the carrier card at the C.O. sending some voltage intermittently causing this unusual behaviour.

I would try calling the pilot number and put it on hold, then call the pilot again so it comes in on line 2 and continue calling the pilot to see if you can duplicate when the problem occurs, could be line 5 or 6 or something, if it happens short out that last line called and retest, or like you said a bad OE at the C.O. It certainly sounds like a Bell problem, could be an adventure getting them to fix it though.
 
I did try the roll over hold test trying to narrow it down to a specific line but no go.
I did not measure mA but found it strange not voltage on idle at all but I did use my backup mini tester.

I ran into a similar problem some time back. If they are analog all the way to the CO (no voip gateway involved), it's probably a split pair somewhere down the line. That's what caused the problem for me. It took a while to convince the telco that that's what the problem was but they eventually tracked it down. The only time you see the problem is when the circuit that is split with gets used in or out which can make it almost impossible for you to track it down because the split could be anywhere........if that's in fact the problem.
 
I thought it strange too about the idle voltage but turns out there is no fibre box on site and just analog lines from the street.

Two of us techs agreed on maybe the split pair issue, I told the client yesterday to have the Bell tech call me to discuss that possibility and what we have done to troubleshoot, they are due PM today.

Yes actually, spoke with them and thought to check back in and report back later this week to see if any further issues.

The contact nearly had a stroke in dealing with the carrier "Bell Canada" after a few escalations and 5 or 6 weeks later it appears to be fixed so far.

"I told the client yesterday to have the Bell tech call me"
The tech said that there was DSL on line 1 and it was on the fiber network, this box for some reason was in the main electrical room instead of tenants suite.
He had said the whole job was done completely wrong and when they installed the box they should have converted all rollover analogs to the fiber network.
I told him that he should check for split pairs anyway and he agreed.

They finally came out again late last week , relocated the fiber box to the suite, re-delivered line 1 as an analog so that the DSL was all alone on the fiber network.

The contact said there was a couple issue but to me it didn't sound like anything to do with the initial issue and I told her lets wait for the end of this week to close the issue.

So will deem it fixed.

Very odd though why they couldn't convert the rest of the lines but the contact mentioned that they did not have the equipment for multiple lines over fibre in their area at the time of the DSL install date (2 weeks prior to when we were involved about the issues).
