All calls going to voicemail

Have an extension that all calls are going to voicemail. DND is not active and there is no call forwading active on it.

Any suggestions where to look for this issue?

Make sure the phone is actually the extension you think it is - if the extension is unplugged, it will go straight to VM.

Then print the extension, and look through it or post it here.
