Alarm 75 on BCM 400 3.0

A customer of mine has a BCM400 3.0. They get an Alarm 75 message on the reception phone every 10 minutes. I've checked the settings and they all seem to be correct, however, the carrier insists that the issue resides on the system. Is there something I might be missing, or is there an issue with the 3.0 system?
 
I have been working on BCM since Beta PRE 1.0 when it was called the Enterpise Edge. Alarm 75 is an issue with the carrier or the connections to the carrier 99.9 % of the time. I would suggest a T1 tester to help prove the case.
 
That alarm indicates that your clock is in free run. Make sure your clock is set to Primary External. If it is, check the cable to the PRI box. Als,check the LEDs on the PRI module. You should have 3 solid green lights.

If all that looks good, you would have to start running some loopback tests, but my guess is the carrier (provided your clock is set properly).
