Agents in call center go to Not Ready at random

In our call center agents have been randomly slipping into Not Ready status. What could be causing this?
 
Are they actually answering the phones? If they are on Non-Forced calls, then the default is to put the agent on Not Ready after a set number of seconds of not answering the phone (usually 15 seconds).
