After a 30 days CP 100/150 will loose external notification

I have a customer out in the field that after 30 days the external notification will stop working. When you go in it says that it is not initialized. This is happening on a CP100 and CP 150. Different sites. Anyone have any experiences with this. I first though of a COS kind of thing but there is not anything visualable.

Remote notification is easy to disable by accident. When a call comes to your cell phone if you hit 2 instead of 1 it turns it off. Not sure if this is what is happening, but it happened to me quite a few times. So often I had to write down the steps to turn it back on so I didn't have to call one of my techs.

Now I make sure to listen to what the canned lady says before I hit any keys :)
 
I had a customer with a similar problem.
I had him delete the mailbox in question and rebuild.
Never heard back from him.
I assume this was mailbox corruption.
Is this problem isolated to a single mailbox?
