afamgt error in event log

What is causing this error in the event log... Am i missing a utility or something. just started happening yesterday.

What model server is this on? It seems like it's just the PERC logging an error, timeouts are reasonably common though and not generally something to panic about. If it's happening a lot now it might be an indication of something about to fail though.

On the PE2600's Dell use a weird split PERC, one channel runs in RAID mode and controller the hard drive backplane, the other channel is (assuming the server was shipped with a tape drive) is configured in SCSI mode and attached to the tape drive. This has been known to cause problems, especially SCSI timeout issues - we've recently installed SCSI PCI cards on some servers to attach the tape drive to so we can get it off the PERC which solves the issue.
 
Thanks for the info. I found out it was a firmware issue. I used the updater package from the download site and it got corrupted. I downloaded the exe to create floppys, re-installed and everything is fine.
