adding the same field twice in report

I need to place the same field twice in to a report but need it to pull different codes from the fields in each column. I would think I need to create a formula to do this...any help is appreciated..

If you're wanting different records from the same table, just add the table again as an 'alias'.

If it's something else, please clarify.
