Adding HBA to Sun Solaris

Hi,

I have connected a server running Solaris 10 x86 to a storage, it looks like trials and errors have generated
many bad disks, I now want to remove them and reboot the system to get a normal situation, please which
command to use ? I'll continue with system file and mounting, please tell me commands syntax or a link
to achieve that.

By the way, I executed "devfsadm -C" and rebooted the system.
The "format" command shows this :

As you can see, it not possible to label /dev/rdsk/c2t0d0p0, what to do ?
Other question, I only have one Lun affected to that server why do I have 3 disks ?
