We would like to add another phone (TN 16 0 0 15) to this 445 group, how would I go about doing this? Many thanks!

First of all, go into LD 20 and print out the TN details for 16 0 0 15. Look for any spare key if you intend to have 445 as an additional DN.

Now go into LD 11 and do something like this

>LD 20
REQ: CHG
TYPE: 2008 (If the phone type is a 2008)
TN 16 0 0 15
ECHG YES
KEY 00 MCR 445 (00 = KEY POSITION)
(Keep pressing enter until you get to REQ)

This should do the trick.

I keep getting a sch0128 when typing the TN... very strange, it definitely does exist.

To the switch it "doesn't exist" if there is nothing programmed on it.
From your post it sounds like you want a new phone. Also, if you want to make it similar to the other TNs then you probably want a different DN on key 0 than 445. To find an available DN.
REQ: prt
TYPE: ludn
DN <-- hit return or, if you know the starting number(s) of the range enter that.

Then you can do
REQ: cpy 1
TYPE: 2008
CFTN 6 14 <-- one of your existing TNs that you want to duplicate
DES JANET <-- this is just letting you know that the DES will be copied. You can change it later if you want
*** MAKE PRINTER READY *** <-- Not important. I guess Nortel thought this was important if you were copying lots of TNs
TN 16 15 (you have an option 11 so you only need to enter the card and unit fields)
DN <-- Your new unique DN for the set
