adding a form to VBA

I want to add a form contaning tahe name of my company and a combobox which populated with the directories containing.cub files on a computer specified.How can I do this Using VBA?

In the VB Editor - Project Explorer, right click and select Userform.

Or use Insert/Userform

And then using the toolbox window, select a ComboBox and then draw it into your user form.
