Add a new Ole object with a form

I have created a database table which has an ole field in the record. What I want to do is create a form that a user can use to enter a new record and ole field.....the ole field relates to pdf locations which are added as a subreport in the report I created.
I do not want the user going to the table itself to enter this data.

Any help is greatly appreciated
