activex in IE causes bad memory reference on close

Hi,
I've got an activex that is deployed over the internet. The application itself runs just fine, but when the user closes Internet Explorer an application error is reported in the form of a message box that reads:
'The instruction at "0x0222e959" referenced memory at "0xffffffff". The memory could not be read. Click OK to terminate the program.'

This happens every time and on many configurations of client machines (note: we only support IE 6). I am pretty sure that somehow my activex is not getting released properly by IE, but I don't know what would cause it. I have checked that every "New" has a corresponding "set x = Nothing", and that the events are fired that run that code, but it doesn't seem to be the problem.

Any ideas will be appreciated.
