ACS R6 forwarding issue

Calls are being transfered to extensions from the AA. The extensions are forwarded to speed dials feature 80-83 on each of the extensions. When call is transferred, call goes out but caller hears nothing (dead air). When caller hangs up, the outgoing line stays lit (off-hook), busying out the line. What should I check?

Have you tried dialing the ext internally & see if it forwards? Also, did this just start happening?? I have numerous customers using this feature & have not experienced any probs.
