This problem has manifested with Acrobat 4, 4.05c and 5. Adobe says there is nothing they can/will do regarding the issue.
When scanning one page at a time - both sides of a duplex document, for example, when I click the 'done' button to indicate that there are no more pages, Acrobat generates an invalid page fault in either kernel32 or twain_32.dll.
If I stack multiple documents in the ADF and let it scan them all in a single pass, there is no problem - only when I have to press 'done' to indicate that I won't be adding any more sheets.
v 1.6.0.3 for Twain_32.dll, running an Epson Perfection 1640su SCSI. Adobe is the only application with any difficulties at all.
