ACD REPT 4

I'm trying to understand the basic ACD Reports of the meridian 11C, I've found information about Rept 1 and Rept 2, but nothing about 4, please could some one explain me the meaning of each column

Besides, is possible to generate a monthly report?

Exist any free aplication or macro to proccess the infomation for humans?
