Account key not working

Setup:

CCMS 6
CCMM 6
CSE1000 5.5
1150E phones
CCT soft-phone

The agents are using the soft-phone, one agent in the group (same default ACDDN) can enter an account code while on a call and it will register in the CC6 database. Other agents cannot. When they enter it, they get the proper response from the soft-phone, but the code never gets logged in the database. Not Ready codes work on all phones and get logged in the database. I afraid this might be a firmware issue on the 1150E, but the two most recent firmware’s from Nortel cause the 1150E's to reboot about 6-7 times a day.

Anybody seen the Account code problem before and 1150E rebooting before?
