Account getting locked

A user having issues with account getting locked out, when i look at the event logs this is what i see

Logon to account :abc
by Microsoft_Authentication_package_V1_0
from workstation:XXX
failed, the error code was 3221225578

I know the error code means mis spelled or bad password, but user already logged on to the machine and suddenly he couldnt access outlook.
What does Microsoft authentication package v1 0 means. Pleae help

Just ran into this one myself.

Look for a service that is utilizing the users local logon credentials. Odds are the user just recently changed their domain password, and the service is using the old one. The user can logon to the domain, but then the service tried using his old credentials and locks his account out before he can open Outlook.

Another possibility is an old open session on a terminal server that was created using the old pw....
 
I have to agree, I have seen this many times when a user is logged on at a second machine somewhere and that session was logged on with an old password.

Check your account lockout policies ...

Most set them to
duration 0 minutes (admin unlock)
threshold 3 invaled
reset 15-30

I had to change my threshold to 5 and reset to 10 minutes do to Outlook's constant background replication. I experienced this on 2 different networks now. Microsoft recomends the threshold to 5 vice 3 for this reason(cant remember where I read this)
