Access Outlook's Name and Addresses and put them on a form

I need to get names and addresses form Outlook and put them into a form. How might I do this.
 
Sort by date Sort by votes
outlook folders can be linked to access just as excell or dbase files
see file menu --> get external data --> on lower left see files of type
