access form on the web

Good day gentlemen

Here is the project I am working on. A simple form accessible via a web page. basically from the form you click a field that launches a query. From the query, you load whatever information you need from the table.

What I am trying to do is a web base database for emergency services agencies to access a data that identifies train crossings and show a link to a maps of the crossing.

The local form, data and maps are done, the only thing is I have not a clue how to put it on the web or if it can be done. Should i do a mde page ???? your help will be greatly appreciated.
